For special power needs over and above the provided 2 plugs, please complete this form and send your check for the full amount payable to the Holiday Inn By the Bay, 88 Spring Street, Portland, ME  04101-3924.  Attention:  Sales Department.  All requests must be received ten (10) days prior to the show.  All late orders will be subject to an additional $15.00 late charge.  PLEASE SUBMIT NO LATER THAN OCTOBER 8, 2012.
